"Unfinished Business" is the seventeenth episode of the fourth season on NBC drama series New Amsterdam, and the seventieth episode overall. It aired on April 26, 2022.
Synopsis[]
After a party, everyone searches for colleagues who didn't show up. Dr. Goodwin finds Dr. Sharpe with blood clots and takes her to the hospital. Dr. Frome contacts Trevor, worried about their relationship. Dr. Castries is unconscious, and Dr. Reynolds and Walsh discuss revitalizing her. Dr. Bloom and Shinwari find Casey with a screwdriver in his neck.
Plot[]
Max runs home to find Helen unconscious on the floor of his apartment, and he rushes her to the ED. Iggy tries to find Trevor. Floyd looks for Mia; he finds her bike, and eventually Mia is hypothermic.
Lauren rings the VA for Ronnie's number. Ronnie answers the phone but doesn’t know where Casey went or even where he is. He tells her they fought, but Casey left. Lauren bumps into Leyla while on the phone. Leyla asks what is wrong, and Lauren tells her Casey is missing and she's going to look for him, to which Leyla replies that she is going with her.
Karen and Ben are let into Elizabeth's apartment by the building super, Elizabeth is found on the floor of her apartment with a head wound.
Helen comes to. She says she is in pain. Max tells her they found Thanatophol in her system and says a number of their drinks were spiked at the karaoke bar. She complains about pain in her leg. Walsh arrives with her obs, and Helen realizes she has the potential for DIC. She needs a scan. Max sees multiple clots. She is going to require a number of surgeries, including a hysterectomy.
Iggy speaks with Trevor. He is in the hospital. He tells Iggy that some good Samaritans helped him on the subway. Unable to remember himself, Iggy asks if anything happened between them. Trevor tells him nothing happened. Iggy is relieved. When Iggy turns around, he notices his husband standing in the doorway. Martin doesn’t say anything other than about their daughter's parent-teacher conference later that day, and then turns around and leaves.
Floyd has Mia in the OR. Her core temperature is extremely low. With warming blankets in place, Floyd instructs Walsh to give her several medications. Walsh states her chart says she is allergic to Dranova, Floyd says to give her Vancomicin instead.
Helen tells Max she wants to forgo a hysterectomy and try a drug called Platazel, which would be less invasive. Max doesn't agree with her decision, but Helen asks him to be her partner, not her doctor.
After Elizabeth is checked out and informed about the Thanatophol, they question who could have done this, and a look of surprise crosses Elizabeth's face. Karen calls the police while Ben updates her brother, then takes off.
Lauren and Leyla walk the streets, yelling for Casey. Leyla informs her about the revocation of her visa and threatens to deport her back to Pakistan unless she challenges it. She needs money for a lawyer, but she is afraid to ask. Just then, they find Casey unconscious with a screwdriver in his neck. When Lauren tries to rouse him, he pushes her hand away and accidentally brushes his neck, knocking the screwdriver out. Lauren compresses it, while Leyla looks for something to seal the wound. To stem the bleeding, she uses the caulk Leyla finds in a dumpster.
Iggy tries to talk to Martin while they head with their daughter to the parent-teacher conference. Martin doesn’t want to talk.
Meanwhile, Floyd talks to Lyn, who just told him the baby is his. He wants her to stay, not move to Colorado with Claude, so that they can raise the baby together. She tells Floyd that she plans to stay with her husband. When asked if there is anyway for the three of them to raise the baby together, Floyd dismisses the idea. She suggests she and Claude raise the baby, which upsets Floyd. Her final, yet reluctant, solution is to terminate the pregnancy.
Helen is telling Max why she doesn’t want a hysterectomy, and as she is about to say she wants his baby, he cuts her off by saying he wants a baby with her. He confesses he was going to propose and then decides now is a suitable time. She says yes. He heads out to get the ring.
Iggy and Martin argue at home. Their children are sitting on the other side of the door, able to hear it all. Iggy says it's harmless, but Martin doesn’t think it is. Martin says he and the children can see Iggy's happiness. Martin thought it was because of him and their lives, but now he realizes it was because of the thrill of flirting with Trevor.
Casey is in the back of the ambulance. His BP is low. He tells Lauren that he knows he is not doing well, but he needs to keep talking, so Lauren tells him about Leyla's situation.
Mia is coding on the table. Her body temperature is too low to shock her heart into rhythm, so Floyd decides to push the Dranova, hoping her allergic reaction will raise her core temperature.
Meanwhile, Elizabeth goes back to the karaoke bar and confronts the bartender. He admits he did it. He claims that the doctors instilled a fear of COVID in everyone. They forced the country to shut down for no apparent reason. Doctors kept their jobs, but he was out of work for 18 months. As Karen and the police arrive to arrest him, Elizabeth passes out.
After a clot breaks loose and lodges in Helen's lung, Max returns to the hospital with the ring.
Across the hospital, Mia is in full anaphylaxis, but her temperature is rising, and Floyd is eventually able to shock her heart into rhythm, just in time to administer an epi-pen.
Overlooking Casey in surgery, Lauren and Leyla talk about their love and money. Lauren says she will help her, but she looks heartbroken, while Leyla hugs her with joy.
Lyn performs an embolectomy on Helen, with Max assisting. Max instructs Lyn to thread the catheter through her heart despite the risk of causing cardiac arrest. It works; however, as the catheter is being removed, Helen crashes. Max shocks her heart back into rhythm. Lyn states that she is stable.
Elizabeth wakes up in the hospital with Karen and Ben by her bedside. She is upset. She claims that people used to view healthcare workers as heroes, but now they perceive them as the enemy. She used to be able to walk home in scrubs, but now she changes into street clothes to avoid attacks. She feels hopeless.
Floyd tells Lyn that his lack of a father made him determined to be a good dad, and he could never abandon his child. But he goes on to say that if Lyn and Claude were the ones raising them, he wouldn't be abandoning the child. He is confident that the child will grow up in a loving home. One day, he longs to reveal his identity to the child and affirm their parents' unwavering love for each other.
Later, Elizabeth wears her scrubs home with pride.
Casey wakes up with Lauren at his bedside.
Iggy sits solemnly with his family, having dinner.
Helen wakes up to Max at her bedside. He tells her they removed all the clots. He displays the ring to her, emphasizing its custom-made nature. She cries and mumbles, realising she cannot speak. Max calls for help.
